District Nursing Team Leader

Job/Role Summary

I assist the Integrated Care Team Manager with the implementation and delivery of an integrated model of care for one of seven geographical teams. I work very closely with social work and occupational therapy Lead colleagues to ensure there is a co-ordinated and integrated approach to allocation assessment, delivery and monitoring of high quality and safe care. As the district nursing lead I am responsible for the delivery of high quality district nursing services and the day to day supervision of the team’s nursing staff. I ensure that all nursing practices comply with the requisite professional standards and NMC Code of Conduct. I also assist the Integrated Care Team Manager with service planning, financial and resource management.

Specific qualifications and experience required

Specialist Practice course in District Nursing. 3 years experience as a District Nursing Sister.
